CCFHS Receives National Recognition
Coos County Family Health Services (CCFHS), Berlin, NH has announced that their entire organization, including medical providers and administrative and technical staff, has won the highest level of recognition by the National Committee for Quality Assurance (NCQA). CCFHS is the first medical practice in Coos County to receive this national recognition!

The NCQA grants awards through its Physician Practice Connections, which has nine standards and three levels of recognition to distinguish Patient-Centered Medical Home Program (PCMH) practices. These standards recognize that the PCMH practices use systematic processes and information technology to enhance the quality of patient care.


This award demonstrates that patients treated at CCFHS can be sure that the team reviews their health histories, and that they will collaborate with both patients and other providers. The team will also treat patients in a language and manner that they understand, and will employ electronic tools to enhance communication and improve patient safety. CCFHS providers, nurses, and other care givers collectively treat acute and chronic medical conditions and monitor health outcomes.


CCFHS has hired Andrea Labonville, RN, to provide case management for patients discharged from a hospital or short term health facility and to work closely with our providers as they develop care plans with these patients. She will also assist patients with medication reconciliation and community resources.


The Patient-Centered Medical Home (PCMH) model has been used to foster stronger patient-physician relationships with a focus on longer-term coordinated care, rather than episodic and complaint-based care. Evidence-based medicine and clinical decision-support tools guide decision making. Information technology (IT) is used appropriately to support optimal patient care, performance measurement, patient education and enhanced communication. Patients are encouraged to participate in their healthcare plan and set goals to maintain a healthy life or slow the progression of chronic disease.


CCFHS uses the PCMH model through collaboration among all stakeholders, inside and outside of CCFHS, to provide excellent preventive and ongoing care to everyone regardless of their economic status. CCFHS has been recognized as providing safe, high quality and effective care that improves patient outcomes and ultimately reduces cost for their patients in Northern New Hampshire.
