- LEAD RN – Colebrook, NH Medical Office
Partial summary of duties:
• Provides triage of urgent/same day or next day appointment needs following triage guidelines, protocols and standing orders
• Facilitates med refills, including obtaining prior authorizations
• Oversees medication samples available for patients
• Oversees vaccines provided to patients of the practice
• Provides on-site clinical support of practice nursing staff and assists in staff evaluations
• Works closely with CCFHS’s Nurse Manager to understand and follow organization policies and procedures and
• assists the local Operations Manager by providing input and oversight of specific clinical processes
Required education and/or experience:
NH RN license required

- Team Coordinator - Colebrook, NH Medical Office
Partial summary of duties:
• Greets and checks in patients, updates patient demographics, verifies insurance information, consent for services, HIPAA information, post payments and co-pays
• Sends out new patient packets, enrolls new patients and assists patients in completing paperwork
• Schedules/reschedules appointments
• Follows up on no-show appointments per CCFHS policies
• Prepares bank deposit slips
• Assists Communication Coordinator to answer calls as needed
• Works closely with CCFHS’s Office Manager to understand and follow organization policies and procedures
Required education and/or experience:
High school diploma or GED equivalent. Demonstrated abilities to be organized/efficient with solid typing skills as well as work well with the public and to operate general office machines, including computers, copiers, and fax machines. Excellent customer service skills.

- Nursing
CCFHS has part-time and full-time positions available for an RN, LPN or CMA to work in our Berlin and Gorham, NH offices.
The staff nurse shall assist the medical provider in the provision of comprehensive primary care services. Responsible for managing, tracking, collaborating and coordinating patient health care needs including: medical, behavioral, oral, emotional, etc. with an emphasis on the whole patient.
Must be available to work some evenings and weekends as applicable.

- Team Coordinator
Responsible for greeting patients; triaging and directing telephone calls, inquiries, and enrolling patients for services; updating patient demographics, insurance information, consent for services, personal contacts, HIPAA information, etc;
